Does sanitizing actually fix the smell, or just cover it?

It depends on where the growth lives. Sanitizer applied over dust does almost nothing. The debris has to come out first. Brushing, agitation, negative air, then the surfaces get treated. Fogging a loaded duct is theater. If anyone offers sanitizing as a standalone spray, ask what gets removed first.

Material decides the method. Bare galvanized trunks in older Lakeland bungalows wipe down and treat cleanly. Fiberglass ductboard in 1960s–1980s block homes is porous. If the interior liner is eroding and shedding fibers, no antimicrobial restores it. That run gets encapsulated or replaced, and we say so rather than spraying it.

Odor often starts at the coil, not the duct. The "dirty sock" smell on heat pumps at changeover is biofilm on wet aluminum. Treating supply branches leaves it in place. Same story with microbial film at supply boots, where attic humidity meets cold metal. That is a moisture and insulation problem the sanitizer cannot solve.
